Donation Request Form
At Ladies and Gentlemen Salon and Spa, we strive to support local non-profit organizations and causes.
Therefore, we have created a guideline and application process. This system will allow us to better serve our community and the high volume of requests received annually.
Due to stringent requirements for financial reporting, the form below must be completed in its entirety and your group/event must meet all stated criteria to be eligible for any donation.
Donation Request Form Guidelines/Requirements
Your request must include your Tax ID# or a copy of your organizations 501(c)3 designation. (A bank established trust may be acceptable in some circumstances, but will need to be confirmed prior to our approval.)
The request must be submitted at least 6 weeks prior to the event. Due to limited processing time, we can no longer accept requests outside of this time frame.
The request must be accompanied with this completed Ladies and Gentlemen Salon and Spa Donation Request Form; no other request written or otherwise will be accepted for consideration.
Due to the high number of submissions we are unable to respond to each donation request. If your donation request is approved, you will be contacted to arrange for pickup. Thank you for your support of Ladies & Gentlemen Salon and Spa!
